The course of the Legato ™ session
Legato ™ is a technology known as Trans-Epidermal Delivery (TED), which takes place in three distinct phases.
There are 1 to 5 sessions depending on the indication and the intensity of the treatment. Each session lasts about twenty minutes and is spaced a month apart.
For the comfort of the patient during the treatment, the doctor applies beforehand an anesthetic cream or suggests a local anesthetic.

The ablative phase
Fractional radiofrequency creates vertical micro-tunnels of 120 microns in diameter every 180 microns of skin, to the dermal-epidermal junction by "microplasma" effect.
This principle of purely ablative action without thermal effect ensures never to stimulate the melanocytes in the basal layer, which is the deepest in the epidermis. Legato ™ can work on all skin phototypes.
Treatment of stretch marks with Legato™ III
As in any fractional technique, areas of healthy skin are preserved around the micro-tunnels to promote an ultra-fast repair of the epidermis.
The doctor sets the density and depth of the micro-channels. This action is absolutely essential when it comes to working the superficial tissues (crumpled, dilated pores, texture, stretch marks, scars, pigmentary lesions, etc.).
The micro-wells will serve as vectors for the active ingredients to directly reach the subcutaneous tissues.
The thermal phase
Thanks to a simple modification of the setting of the Legato ™, the doctor transforms the "micro-plasma" action of the radio frequency into an essentially thermal and deep action. This gradual rise in temperature in the connective tissue has the effect of generating inflammation of the fibroblasts, forcing them to create collagen and elastin.
This action is absolutely necessary when the doctor works on the ptosis, whether it is on the face (the oval in particular) or to defibrose the deep tissue in the treatment of stretch marks and scars.
Treatment of wrinkles with Legato™ III
The thermal phase is carefully avoided when treating particular skin marks such as keloid or hypertrophic scars or melasma in which the vascular network or melanocytes must not be stimulated by heat.
The penetration of active molecules
The doctor deposits in small drops on the area to be treated, a mixture of active products intended to pass into the dermis (non-crosslinked hyaluronic acid, retinoids, vitamins, peptides, organic silicon, etc.) as NCTF® 135 HA FillMed.
By simple contact on the skin, ultrasound at a very particular frequency (about 50 Khz), alternating pressures and depressions (phenomenon of sonophoresis), act as an acoustic jackhammer and will force the penetration of compounds to the deep tissue.
There is some rednesses lasting according to the intensity of the treatment: two to seven days maximum. Light crusts may also appear.
To prevent and mitigate inflammation, an LED session is done before and after a Legato ™ treatment.
The patient should not be exposed to the sun for three to four weeks. It is also important to apply a repairing balm that promotes skin repair for three to four days.
The patient notices an improvement from the first session but the results are optimal two to three months after the end of the treatment so the skin collagen regenerates.
